Nurse Practitioner OR Physician Assistant
Go Docs Go, a physician house call practice, is seeking both part-time/full-time Nurse Practitioners and Physician Assistants as primary care providers to provide at home care to our geriatric and disabled patient base, primarily Medicare recipients, residing in the area of Hillsdale County. The Go Docs Go practice uses a physician / nurse practitioner team model and provides exceptional care by bringing compassionate providers directly to your door.
Summary:
The Nurse Practitioner or Physician Assistant will conduct primary care patient visits in private homes, group homes and independent and assisted living facilities. Our practice requires an individual who can conduct examinations independently with excellent clinical competence. Patients are visited approximately every four to six weeks in an effort to promote health maintenance and avoid unnecessary hospital visits. The practice will emphasize health promotion and maintenance, disease prevention, and the diagnosis and treatment of acute and chronic health problems. The Nurse Practitioner will work collaboratively with the physicians, hospitals, skilled nursing facilities, patient's residential facility, as well as clients in their own home to develop care protocols.
ESSENTIAL DUTIES AND
Responsibilities:
o Conducts medical evaluations of predominantly elderly, frail, chronically ill home bound patients in their home environment.
o Documents visit completely and accurately via EMR at the time of the visit for medical record and billing purposes.
o Develops a treatment plan in collaboration with the physician, patient, and significant others.
o Evaluates patient medication and writes/orders prescriptions as needed.
o Performs minor surgical procedures when required.
o Refers patients to physicians as appropriate for consultation or for specialized health resources and treatment.
o Maintains current knowledge of and promotes compliance with all federal, state, and local regulations. Keeps fully cognizant of current Medicare, Medicaid, and third-party reimbursement regulations and changes and ensures compliance.
o Maintains patient and staff privacy and confidentiality at all times pursuant to HIPAA Privacy Final Rule.
o Meets minimum productivity requirements based on practice standard.
IDEAL CANDIDATE WILL HAVE:
o Minimum education: Masters required
o Adult, Geriatric, Family or Acute Care specialties preferred
o 1
years experience as NP or PA; Home Care nursing experience a plus
o Active, unencumbered nursing license
o Nurse Practitioner certification from either the ANCC or AANP, a plus
o Must be able to work independently in the field, driving and navigation skills
o Must have working knowledge of computers, cell phone, fax machines, internet
o Familiarity with ICD-9 and CPT coding
o Superior communication skills required
o Excellent organizational skills with ability to multi-task
o Reliable transportation
